All application materials must be received by the Division of Endodontology no later than September 1st of the year preceding July matriculation.
The following application materials must be sent to the Division of Endodontology (address is listed below) for consideration for admission:
1. Completed application form (at the end of this form is the area for the applicant's personal statement.)
2. $75 application fee payable to the University of Connecticut Health Center.
3. Transcripts from all undergraduate and professional schools.
4. Three letters of recommendation and certificate of class standing.
5. National Dental Board examination scores.
6. Copy of curriculum vitae.
7. For applicants whose native language is not English : TOEFL with a minimum score of 550 for paper-based exam of 215 for computer based exam.
